Address, voice/fax numbers, and transcript from the school in which the student is transferring from

Grades at time of withdrawal from previous school if transferring while school is in session

If student is receiving special services any paperwork would be helpful to the school for class placement.

In cases of divorce or separation, custodial papers signed by judge noting legal guardianship of student
